In-Year Application 2023/24
If you wish to make an application, for your child/children to join Servite RC Primary School, please complete the following:
- School's Supplementary Information Form (Click here for form)
- Certificate of Catholic Practice (you can obtain the certificate from the Priest at the Parish where you normally worship)
- Baptism Certificate
- Proof of home address (Which includes: a council tax bill, utility bill or bank statement dated within the last three months)
Once completed please return to the school office.
